Step 4 - Bring membership application, quiz results, proof of vaccinations(*), current dog license, and payment to the Parks and Recreation Department at Hillside Monday-Friday 8:00am to 4:30pm
(*) - Applicants must provide a copy of current dog license and a copy of current vaccinations of rabies, DHLPP and bordatella for each dog registered. A parasite preventative is recommended.
